Medical Information. In order for medical professionals to give medical opinions regarding your health, you may be required to provide us with past and current medical records, description of symptoms, a pictorial representation of the issue, a medical history and life-style descriptions.

Correspondence. If you correspond with us via email, we may gather in a file specific to you the information that you submit in order to communicate with you effectively and respond to any of your inquiries. We maintain certain Personal Information about you as
long as reasonably necessary for us to complete our service with you, or as may be required by law.

URL and IP addresses. Company collects information about users’ utilization and navigation of our Site. This information helps us to design our Site to better suit our users’ needs. For example, our Site will track the URL that you visited before you came to our Site, the URL to which you next go and your Internet Protocol (IP) address. We use your IP address to help diagnose problems with our server and to administer our

If you would like to delete your account, we will delete all information connected with your account from our servers. However, we can not guarantee that this information will be deleted from the servers of the third-parties.

Additional Information About This Privacy Policy:

The policies indicated in this Privacy Policy will remain effective, even if the member’s coverage is terminated, to the extent we retain information about a member. We may change this Privacy Policy at any time and will inform you of any changes as required
by law or regulation.

Service Use Termination & Refund

You may terminate your use of the Services at any time by not using the Services. If you wish to terminate your annual health plan, you can do so by contacting us via email at tbilisi@amcenters.com. If Enrollee terminates Plan within one month of enrollment, payment for AMC Annual Health Plan shall be fully refundable unless actual Enrollee visits which are to be charged at regular AMC rates and deducted from the refundable amount. The AMC Health and Wellness Managed Care Plan is non-refundable after one month of enrollment time unless mutually agreed to in advance.

CORPORATE TESTING FOR BUSINESS TRAVEL

If you require numerous employees to be tested on the same day for an upcoming business trip, we can organise an onsite workplace testing session.

How long does it take to get back results?

Results for PCR Testing takes up to 24 hours to process. We also offer rapid testing for those requiring same-day results.

COVID-19 Fit to Fly Travel Certificates available at AMC

Upon request, we may provide a Covid-19 travel certificate for travel purposes, verified by a doctor to provide proof of a negative test result, at no additional cost. When required, a certified Covid-19 travel certificate can be presented to show you’re safe to fly and enter a country.
